Parent/Guardian Requirement
A parent or legal guardian must remain on-site and in attendance for the entire hour long reading. Children may not be dropped off.
Photography & Social Media Release
I authorize Region Six, its staff, volunteers, sponsors, and partners to photograph, video record, and use my child's name, image, artwork, and likeness in print, digital media, websites, promotional materials, news releases, and social media platforms. I understand these materials may be used now or in the future.
Liability & Medical Authorization
I release Region Six, its staff, volunteers, sponsors, and affiliates from liability for ordinary negligence associated with participation, except as prohibited by law. If I cannot be reached, I authorize emergency medical treatment for my child if necessary.
